Qualifications (mandatory):

- Manage patient self-discharge and drop-off, working proactively to identify and address factors contributing to hormone health patient attrition.
- Document hormone health patient interactions and maintain accurate, up-to-date patient records in the electronic health record system.

- Capture and report on key analytical data related to hormone health, including prescribed vs. actual treatment adherence, drop-off rates, self-discharges, interdisciplinary referrals, and other relevant metrics to inform clinic performance and patient outcomes.
- Schedule hormone health patient appointments efficiently when necessary, managing practitioner calendars and coordinating with patients to find suitable times.
