Utilizing Comprehensive Plans of Care

At Red Leaf Wellness we work with two related but distinct things. The first is your clinical care plan: the assessment, treatment approach, and interventions you bring as the clinician responsible for a patient. The second is the patient's overall Plan of Care, which is the shared roadmap the whole team uses to keep the patient progressing, coordinate across disciplines, follow up consistently, and stay proactive rather than reactive. Tell us how you develop and manage your own clinical care plans. Then, just as importantly, tell us how you contribute to and work within a shared Plan of Care. How do you help keep patients on track, flag when someone is starting to fall off, and keep the wider team informed so the patient never slips through the cracks?
