You send a referral

Fax your completed referral letter using our standardized template. Include patient details, current medications, reason for referral, and desired outcome.

We contact your patient

Our admin team books the initial NP assessment and assigns consent and intake forms. Consent must be completed 48 hours before the appointment.

NP assessment & orders

The NP personally assesses your patient via video, establishes a clinical relationship, orders required labs or imaging, and builds a plan aligned with your referral goals.

Report faxed to you

A formal clinical note is sent to your practice consistent with the referral arrangement you set. Serious or critical findings trigger immediate follow-up with the patient.

The Nurse Practitioner Specialist Referral program is structured as a true consultative referral model – consistent with regulatory college standards for referral, consultation, and continuity of care. Depending on your patient’s needs, you can refer for a one-time intervention, ongoing co-management, or a full transfer of a specific area of care.

Our program solves the most common ceiling in natural medicine – your patient is ready for prescription management or hormone therapy, but your prescribing scope doesn’t cover it. This program bridges that gap cleanly, with the NP working within the referral structure you define.

Appointment fees cover the full block of clinical time reserved for your patient. This includes time spent directly with the NP as well as time used to review records, manage prescriptions, complete documentation, and carry out other clinical tasks that support your patient’s care. Not every minute of the duration is a face to face clinical encounter.

Common questions from referring clinicians

No. External referrals from any licensed practitioner are accepted. The process is straightforward: a standardized referral template captures the clinical question, relevant history, and co-management expectations upfront, so the NP has everything needed to assess your patient and return a consultation note to you.

Patient name, DOB, contact details, healthcare number, current medications, known allergies, past and current medical history, reason for referral, desired outcome, and the report-back format you’d like. Our template guides you through all of this.

It depends on the referral. For most referrals, yes the NP functions as a consultant, assessing the patient for the specific clinical question referred and working alongside you as their primary provider. In some cases, such as when a patient is transitioning a specific area of care to the NP’s ongoing management, the NP assumes clinical responsibility for that area while you remain their provider for everything else. You define the structure when you refer, and the program accommodates all arrangements.

The report-back structure depends on the referral arrangement you set. For co-managed patients, your practice receives a formal clinical note after each NP visit covering the clinical question assessed, findings, any investigations ordered, and the recommended follow-up plan. For a full transfer of care, you receive an initial consultation note and are notified of any material clinical developments. For one-time episodic referrals, you receive a note at the conclusion of the episode. Serious or critical findings always trigger immediate follow-up with the patient regardless of care model.

Yes. As a regulated nurse practitioner, when appropriate, the NP can order testing through the publicly funded lab system. This is a byproduct of the NP’s scope of practice, not the purpose of the referral. Patients are referred to the NP because the clinical question warrants an independent specialist assessment – the lab order flows from that assessment, not the other way around.

Patients are directed to contact you first with any questions or concerns about their prescription or care. If you determine a change to the care plan is needed, you can send us an updated note and we will act on it. For urgent concerns, patients are advised to seek emergency care as appropriate.

Dr. Esnielle Brooks is a family physician and Menopause Society Certified Practitioner who serves as Medical Director at Red Leaf Wellness. In this role she provides clinical governance and direction for our menopause and hormone health programs, working alongside our Nurse Practitioners and Naturopathic Doctors to keep our care pathways evidence-informed, current, and consistent across every clinician who delivers them.

Dr. Brooks earned her MB ChB at the University of the Free State in South Africa in 2007. She spent five years in rural South African practice, where family medicine, emergency medicine, obstetrics, and anesthetics all fell within a single day’s work. In 2013 she joined Norwegian Cruise Line as a senior physician running a shipboard medical centre, and in 2015 she and her family settled in Alberta. She has practised in Red Deer since 2020 and holds registration on the General Register of the College of Physicians and Surgeons of Alberta.

Her focused practice is women’s midlife health. She is the founder and lead physician of The Menopause Clinic in Red Deer, where her work centres on perimenopause and menopause, hormonal and non-hormonal symptom management, and vulvovaginal and genitopelvic concerns including vaginal dryness, dyspareunia, and vulvodynia. She has pursued menopause training well past the requirements of general family practice, including certification through The Menopause Society, IMPART certification through the International Menopause Society, menopause coursework through the SOGC and Mayo Clinic, and the ISSWSH course on genitopelvic pain.

At Red Leaf Wellness, Dr. Brooks’s work is largely behind the scenes and in service of the clinicians you actually see. She provides clinical direction on protocol and care pathway development, offers curbside consultation and mentorship to our clinical team, advises on compliant care delivery models in Alberta, and contributes to patient education materials and presentations.

Dr. Brooks holds her Medical Director role at Red Leaf Wellness as an independent contractor. She is not currently accepting patient appointments at Red Leaf Wellness; her role here is clinical governance and advisory. Patients seeking care are welcome to book with any of our Nurse Practitioners or Naturopathic Doctors.
