As a Patient Care Coordinator for Hormone Health at Red Leaf Wellness, you will conduct discovery calls, review care plans, answer clinical questions, provide treatment recommendations, ensure consistent care, manage aftercare follow-ups, document interactions, analyze data, schedule appointments, contribute to process improvements, collaborate with practitioners, participate in a mentorship program, and perform other duties to support our patients’ hormone health journeys, including BHRT. As a key member of our team, the Patient Care Coordinator plays a crucial role in ensuring our patients receive exceptional care and support.

Key Responsibilities:

  • Conduct brief telephone or video calls with prospective patients to answer non-clinical questions about fees, processes, and recommended practitioners, focusing on our hormone health services, including BHRT.
  • Review practitioner plans of care specifically for hormone health, identifying unmet clinical integrative health needs and coordinating necessary referrals, labs, and supplements related to hormone balance.
  • Answer basic and intermediate clinical questions on behalf of practitioners, providing patients with the information and support they need to navigate their hormone health care plans effectively.
  • Provide general recommendations for a course of treatment, including lifestyle adjustments and natural therapies related to hormone health, under the guidance of practitioners.
  • Communicate and coordinate with patients’ practitioners to ensure consistent hormone health care, under the direction of their practitioners.
  • Conduct aftercare check-ins and follow-ups, ensuring hormone health patients are adhering to their treatment plans and addressing any concerns or challenges that arise.
  • Manage patient self-discharge and drop-off, working proactively to identify and address factors contributing to hormone health patient attrition.
  • Document hormone health patient interactions and maintain accurate, up-to-date patient records in the electronic health record system.
  • Ensure adherence to privacy guidelines and health information maintenance protocols.
  • Capture and report on key analytical data related to hormone health, including prescribed vs. actual treatment adherence, drop-off rates, self-discharges, interdisciplinary referrals, and other relevant metrics to inform clinic performance and patient outcomes.
  • Schedule hormone health patient appointments efficiently when necessary, managing practitioner calendars and coordinating with patients to find suitable times.
  • Contribute to the ongoing development and optimization of hormone health patient care coordination processes, identifying opportunities for improvement and implementing data-driven solutions.
  • Work closely with hormone health practitioners, clinic staff, and other stakeholders to ensure seamless patient care coordination and a positive patient experience.
  • Participate in a mentorship program during onboarding, provided at no cost to enhance professional skills and career advancement.
  • Engage in personalized coaching, hormone health patient care coordination skill enhancement, and professional growth through the program.
  • Perform other related duties as required, ensuring the clinic’s needs are met promptly and efficiently.

Must-Have Qualifications:

    • Experience or certification in hormone health, endocrinology, or related fields.
    • Strong competency in productivity tools such as Google and/or Microsoft Office Productivity Suites (Word, Excel, PowerPoint, Outlook) and project management software like Asana. Certifications in these areas are a significant asset.
    • Proficiency in clinic management software and tools, including Electronic Health Records (EHRs) such as Jane, practice management software, and patient communication platforms like DemandHub.
    • Excellent communication, organizational, and problem-solving skills.
    • Ability to multitask and prioritize in a fast-paced environment.
    • Strong interpersonal skills and a patient-centric approach to care.
    • Flexibility to work days, evenings, and/or weekends as necessary.
    • Self-motivated, responsible, punctual, professional, and client-focused.
    • Dedication to patient success and a passion for integrative healthcare.
    • Current CPR certification.
