1. Eligibility for Referral Credit
- This referral program is exclusively available to current patients of Red Leaf Wellness who have completed an Initial Consultation as part of the Hormone Health Program.
- To be eligible for the $25 referral credit, the referring patient must submit the referral claim before the referred individual starts any services at Red Leaf Wellness, including the Initial Consultation.
- The referral must be claimed via the provided form, where the referring patient will submit their name, phone number, email address, and the name of the referred individual. No retroactive referral claims will be accepted after the referred individual has started our services.
- Referral credits are granted only after the referred individual completes their Initial Consultation within the Hormone Health Program.
2. Use of Referral Credit
- Referral credits are non-transferable and can only be applied toward any future consultation, treatment, service, or product offered at Red Leaf Wellness.
- Credits cannot be redeemed for cash.
- Credits will be automatically applied to the referring patient’s account after the referred individual completes their Hormone Health Program Initial Consultation.
3. Limitations on Referral Credits
- A patient may refer multiple friends or family members to the Hormone Health Program, but there is a maximum of three referral credits ($75 total) that can be earned in any 6-month period.
- Red Leaf Wellness reserves the right to cap the total referral credits any one patient may receive for Hormone Health services to maintain fairness and prevent program abuse.
- Referral credits must be used within 12 months of issuance. Unused credits will expire after this period.
4. Patient Responsibilities
- By participating in the referral program, the referring patient agrees to act in good faith and refer individuals who may genuinely benefit from the Hormone Health Program services offered by Red Leaf Wellness.
5. Confidentiality and Privacy
- Red Leaf Wellness respects the confidentiality of all patients. Participation in the referral program requires the referring patient to provide explicit consent to correlate their referral with the referred individual. This is done through a form where the referring patient will voluntarily provide the following information:
- Their name, phone number, and email address (to ensure two points of contact in case one fails).
- The name of the person they are referring.
- The information provided in this form will only be used to manage the referral program and ensure proper tracking of referrals and credits. Red Leaf Wellness will not share the personal or health information of either the referring or referred patient without explicit consent, except as required to administer the program.
- Red Leaf Wellness complies with Canadian privacy laws, including PIPEDA, PHIPA (in Ontario), PIPA (in Alberta and BC), and other applicable provincial privacy legislation to ensure that personal health information is handled in a secure and compliant manner.
6. Compliance with Provincial Regulatory Guidelines
- This referral program is designed in accordance with the ethical guidelines of the regulatory bodies overseeing Nurse Practitioners and Naturopathic Doctors across Canada, including but not limited to:
- College of Complementary Health Professionals of British Columbia (CCHPBC)
- College of Nurses of Ontario (CNO)
- College of Registered Nurses of Alberta (CRNA)
- College of Naturopaths of Ontario (CONO)
- College of Acupuncturists of Alberta (CAA)
- The referral program complies with professional standards for patient incentives, transparency, and non-coercion. Referral credits are modest in nature and do not influence or affect the quality or type of care provided within the Hormone Health Program.
- Red Leaf Wellness clinicians are bound by their professional codes of ethics, ensuring that the referral program does not interfere with the autonomy of clinical decision-making or the well-being of patients.
- In the event that any aspect of this referral program conflicts with the ethical or regulatory requirements of the relevant professional bodies, those regulations and guidelines will prevail. Red Leaf Wellness is committed to upholding the highest standards of patient care and professional integrity in all aspects of its services.
7. Changes to the Referral Program
- Red Leaf Wellness reserves the right to modify, suspend, or terminate this referral program at any time without prior notice. Any changes to the program will be communicated through official clinic channels, including the website and direct patient communications.
- If the referral program is modified, any credits earned prior to the change will be honored, and their use will continue to be governed by the terms in place at the time of issuance.
8. Program Abuse and Fraud Prevention
- Red Leaf Wellness reserves the right to disqualify any participant who attempts to exploit the program or engage in fraudulent activities, including but not limited to:
- Referring individuals who do not have a legitimate interest in receiving care through the Hormone Health Program.
- Providing false testimonials.
- In cases of suspected abuse, Red Leaf Wellness may revoke credits and take further action as deemed necessary.
9. Limitation of Liability
- Red Leaf Wellness is not responsible for any technical issues, platform malfunctions, or other problems that may prevent patients from submitting referrals.
- Credits are applied at the sole discretion of Red Leaf Wellness, and the clinic reserves the right to review and approve all credits before they are issued.
