Red Leaf Wellness Public Testimonial Program: Terms and Conditions

Our Public Testimonial Program encourages patients to share their experiences with Red Leaf Wellness on public platforms. We value your honest feedback, as your stories help others make informed choices about their health and wellness. Please review the terms and conditions below to understand how the program works.

Consent to Disclosure

By participating in the Public Testimonial Program, patients consent to allow Red Leaf Wellness to publicly disclose their name and general location alongside their testimonial, for the purposes of transparency and authenticity.

Public Testimonial Quarterly Draw

  • Quarterly Draw for Testimonials: Patients who post a visible, genuine, and honest testimonial about their experience with Red Leaf Wellness on a public platform are entered into a quarterly draw for a dining experience up to $200 at a restaurant of their choice (as long as the restaurant allows for Red Leaf Wellness to prepay or purchase a dining gift card). This is our way of saying thank you for sharing your story and helping others discover our care. Together, we’re creating a community that celebrates wellness and the spirit of giving back.
  • No Cash Value: The prize has no cash value and is valid only for a single dining experience up to $200 total.
  • Transparency in Testimonials: Red Leaf Wellness encourages transparency in testimonials and welcomes all feedback—positive, neutral, or negative—as long as it reflects a true experience with our clinic.

Eligibility

  • Verified Patients Only: Only verified patients who have received services at Red Leaf Wellness are eligible to participate in the Testimonial Program.

Patient Responsibilities

  • Platform Compliance: Patients providing testimonials must ensure that their reviews comply with the terms and conditions of the platform used (e.g., Google, Facebook), including any disclosure requirements indicating that they may be entered into a draw as a result of their feedback, if required by the platform.

Confidentiality and Privacy

  • Privacy Assurance: Information provided in the testimonial form will only be used to manage the Testimonial Program and ensure proper tracking of entries. Red Leaf Wellness will not share personal or health information without explicit consent, except as required to administer the program.
  • Compliance with Privacy Laws: Red Leaf Wellness complies with Canadian privacy laws, including PIPEDA, PHIPA (in Ontario), and PIPA (in Alberta and BC), ensuring that personal health information is handled securely and in compliance with applicable regulations.

Compliance with Provincial Regulatory Guidelines

  • Ethical Standards: This program is designed in accordance with ethical guidelines from regulatory bodies overseeing Nurse Practitioners, Naturopathic Doctors, and other healthcare providers in Canada, including:
    • College of Complementary Health Professionals of British Columbia (CCHPBC)
    • College of Nurses of Ontario (CNO)
    • College of Registered Nurses of Alberta (CRNA)
    • College of Naturopaths of Ontario (CONO)
    • College of Acupuncturists of Alberta (CAA)

Patient Care Integrity

The Testimonial Program is designed to encourage honest feedback, with each participant eligible for up to three entries per quarter into the prize draw, provided that each testimonial is unique and posted on a different public platform (e.g., Google, Facebook, Yelp). Testimonials must reflect a true experience and be tailored to each platform, not duplicated or repeated across entries.

The prize is modest in nature and does not influence or affect the quality of care provided at Red Leaf Wellness. Our clinicians adhere to professional codes of ethics, ensuring that this program does not interfere with clinical decision-making or patient well-being. In the event of any conflict between this program and ethical or regulatory guidelines, the requirements of relevant professional bodies will prevail.

Draw Details and Notification

  • Draw Schedule: The quarterly draw will take place on the last business day of each quarter, or on the date specified in our current event.
  • Notification of Winners: Winners will be notified via email or phone within five business days following the draw and will be announced on our social media channels.

Use of Testimonials

  • Marketing Use: By participating, patients grant Red Leaf Wellness permission to use submitted testimonials for marketing or promotional purposes, including on social media, website features, and other public platforms.

Changes to the Testimonial Program

  • Modification Rights: Red Leaf Wellness reserves the right to modify, suspend, or terminate this Testimonial Program at any time without prior notice. Any changes will be communicated through official channels, including the website and direct patient communications.
  • Draw Entry Honoring: Draw entries earned prior to any program changes will be honored and will continue to be governed by the terms in place at the time of entry.

Program Abuse and Fraud Prevention

  • Prevention Measures: Red Leaf Wellness reserves the right to disqualify participants who attempt to exploit the program or engage in fraudulent activities, including but not limited to:
    • Submitting false testimonials.
    • Engaging in activities that do not align with a genuine intent to share true experiences.
  • Revocation of Entries: In cases of suspected abuse, Red Leaf Wellness may revoke entries and take further action as necessary.

Limitation of Liability

  • Technical Issues: Red Leaf Wellness is not responsible for any technical issues, platform malfunctions, or other problems that may prevent patients from submitting testimonials.
  • Draw Entry Discretion: Draw entries are applied at the sole discretion of Red Leaf Wellness, and the clinic reserves the right to review and approve all entries before they are confirmed.
